COMPLAINTS QUESTIONNAIRE

We would like to ask you some questions
to find out about children and young people making
complaints.

The questions below will help us find out if you have
been given good information about your right to
complain and how easy or hard it might be to make
a complaint.

What you tell us will help the Welsh Assembly
Government understand more about what
children and young peoples’ views are on complaints.

Any personal details like your name will be kept
strictly confidential. This means no one outside
of Voices From Care (Cymru) will know what you
have said in the questionnaire. You also do not
have to put your name on it if you do not want to.
